Provides sums of up to £6,000 for fractures, ligament tears, tendon ruptures and dislocations.
You are classed as a smoker if you have smoked or used any tobacco-related products (including nicotine patches and e-cigarettes) within the last 12 months.
You must be aged between 18 and 89 to apply.
